Application Process Workflow

In order to understand how our application process works, you will need to review the flowchart/workflow shown.

At each stage of the process, you will be prompted and guided on what to do.

- You first read & fill the Authorization and Consent For Release and Disclosure (ACRD)

- Complete the Application Package (AP)

- Fax copies of your CPR and First Aid Certification, Licenses (LPN/RN/CNA Certification - whichever is applicable), Driver's License, Social Security Card and TB Test (Chest X-Ray if applicable) to 678-964-2217.
